Something noteworthy is that I looked through the forums and found a post with the same issue here.
However, the solution was marked as only a visual bug and that it should not affect the outcome of a craft. As seen in my screenshot, I cooked 1050 Hearty meals, this was done using 4 out of 5 pieces of the Chef’s set, giving me a +16% chance of additional items. Not to get a single bonus item out of 1050 is improbable.

If i recall correctly hearty meals doesn’t proc bonus results unless the fort that has yield is active. It’s been like this for a very long time. It definatley works on any attribute food because I cook almost everyday for people with my trophies and clothes.

I am almost certain the first light fort was ours when I started crafting. I can’t guarantee this though since I did not take a screenshot of the map unfortunately. I’ll test again when we control the fort!
Seems weird for the bonus not to apply on certain meals, I assume this can still be a bug, just not as severe as I first thought if it works on attribute foods, just like you say..

Whether it is a bug or not, i dont know. When wearing cooking armor it says 0% chance so seems it is intended.

It’s the same for lvl 100 and 150 recovery food.

That is true. However, when looking through the forums I found another post from November last year about a similar issue (that the bonus percentage are too low and incorrect). This post was marked as solved with the comment that there’s an UI bug showing a too low percentage and that this was only a graphical issue and all bonuses are applied properly.
This made me not trust the in-game bonus percentage.
It will be interesting to get some further insight in on this and why the bonus are not applied for certain meals..

Just to be clear there are a lot of crafted items that produce no bonus yield. For example proficiency potions, honing stones, oakflesh balm, etc. I don’t find it too strange that a very commonly consumed and ridiculously cheap to make food doesn’t get the benefit of the accumulation gear.

What is more concerning is that with cooking accumulation you usually get your bonus and then some… but then you make 10 asmodeum with 30% bonus and you don’t get a single bonus craft.
