Has been happening to me since the latest patch also. Only way to fix it is to close the game.

closing the game didn’t work for me, I had to die

Sorry to hear that pal, that isn’t good. I tried going to the main menu and switching characters and that wouldn’t work for me, only the full close.

Hope they fix this one soon.
