What are the steps to reproduce the issue as you experienced:
I have found *three* ways to stack LIFESTEALING simultaneously
Primary Weapon (Sword for testing)
Offhand Shield (Used with sword)
Special Named Amulet (Not to be confused with Ring’s Leaching)
Below are my tests;
Sword (4.9%), Shield (No), Amulet (No) with 1280 Damage
→ It healed me either +63 or +64 HP (I guess due to rounding )
Sword (4.9%), Shield (4.9%), Amulet (No) with 1354 Damage
→ It healed me either +67 or +68 HP
→ It should be double that as 4.9% + 4.9% = 9.8% (+132.692 expected healing)
Sword (4.9%), Shield (4.9%), Amulet (4.0%) with 1354 Damage
→ It healed me either +67 or +68 HP
→ It should be almost 3x that as 4.9% + 4.9% + 4% = 13.8% (+186.852 expected healing)
For Science;
I tried using LEECHING and Trenchant Recovery
→ LEECHING and TRENCHANT RECOVERY seem to work with LIFESTEALING
I wore a leeching Ring (+5%), Trenchant Axe (+28%), and the Lifestealing Amulet (+4%)
I did a heavy attack for 2,448 damage and healed +909 HP
5% + 28% + 4% = 37%. 37% of 2,448 is 905.76
I assume it’s off by a few points due to rounding

Is lifesteal on amulet and shield a new thing? You can’t craft them.
There is a difference between perks lifestealing and leeching.

Lifestealing can be found on shields.
For example here is a BoE named drop you can find;

intresting… i know i can sustain quite a lot with leeching flurry on armor, lifesteal on rapier and leeching on ring.
which amulets?
looks like there is two. fortunes favor and hematie one right?
at 450 is that even worth it?
for science why not try with only the amulet on as i think that is probably the issue.
I have tested with just amulet and it works as intended.
But, you are right that it is likely not worth it to sacrifice the GS for a gimmick amulet. This is just exploring what perks stack and which don’t.
Amulets can have +% incoming healing which DOES stack with leeching/lifestealing/trenchant recovery.

Sword + Shield lifestealing is never supposed to stack in the first place. Leeching+Lifestealing does (ring+weapon).
Skill perks on Armor never stack and perks on weapons never stack (sword+shield is really the prime example of this).
So your example is perfect, perks of the same name wouldn’t stack on the different pieces. Take Keen for example… there is Keen (weapon) and Keen Awareness (ring). Having Keen on your sword AND shield would NOT stack it, but having Keen (weapon OR shield) and Keen Awareness on ring would stack the +11% chance to get a critical hit.
Some do stack right?
Luck on weapons, luck in sockets, and luck on armor?
I’m thinking of testing out refreshing for myself next.

Skill perks on Armor never stack
Refreshing absolutely stacks. Luck, Refreshing Evasion, etc.

I’m going to also try and see if it’s possible to get trenchant recovery on a shield and see if that stacks with sword.
Or lifestealing on shield and trenchant on sword since I only had a trenchant recovery axe atm
