While the chapter and page names are correct for Shattered Moutain.
Only thing i’d add to your list:

That is correct, they have different titles. I would argue that Brightwood’s titles are the correct ones and this is an oversight.
It still stands that this is infact the exact same lore entry, as the text on each page is the same.

This one seems to be fixed, and you cant charge on it sadly 
We were able to spot the Garden of Genesis page 2 (A Strange Substance) behind a wall, but did not see a way to get to it. Has anyone managed to pick it up?
We could make out the barest hint of blue page glow in that corner by the wall, but couldn’t get closer to the area than this.
If you’re interested get the King of Emerald: Silence in WW now before the next patch, they’re nerfing charge and burnout which is required to get it.
Kinda gave up on these pages… like some of them aren’t ingame or cant be taken, but hey they added new ones that cant be picked either… fun way to learn the lore
They fixed all the pages added in the previous patch and added a few more (missing) ones.
Anyone managed to find the new ones in Mourningdale?
