I have seen some discussions on this at the beginning of April. It feels as though there is something wrong with the loot tables in the Tempest Heart dungeon. It seems like a lot of players have a named item that refuses to drop for them. Mine appears to be the Blackguard’s Hatchet. I have killed Uzret 60+ times without seeing this drop. I have acquired most of the low drop rate named items in this game that are considered BIS and have never seen such a low RNG to this point which makes me think that there is something wrong with the drop tables. Many other people have ran the dungeon an extreme amount of times and are also appearing to not be able to find one specific item.
