I enter behind the Soulwarden Seal, the quest starts without me initiating it. I kill the boss and have to start the ritual again. That option to start the ritual never goes away, even during the fight. Then I start the ritual and all repeats forever. I’m not able to finish the quest. Additionally, I have multiples of the quest item in my inventory. See the screenshots. - Is this a bug?

Just try to do the quest. Seems like everyone has this issue and can’t finish the quest right now. - When did this happen? (Date and Relative time; please include your timezone)

Update: It’s possible to finish the quest. When you kill the boss, begin the ritual and wait still, no moving at all, until the quest is done.
